Each year, insurance companies are required to report a medical loss ratio (MLR) to state and federal agencies. Groups insured with us at any point in 2024 will receive an email containing the MLR survey to complete. 

Why is this important?
Completing the survey each year and providing information on a group’s size helps us determine which groups qualify for rebates. Please note that rebates are assessed annually and are not guaranteed.

Group Letters and Surveys
Surveys will be sent to the group leader's email address on file. There are two types of surveys:  

• Non-ERISA group letter and survey
• ERISA group letter and survey

We will send an email to group leaders providing details on completing the survey.

How To Submit Completed Surveys
Groups can return completed surveys via the following:

Group leaders should receive a confirmation email within five to seven days after submission, indicating that the MLR survey has been received and is being processed.
